Inflammation diseases of the periodontium are caused by microorganisms and their toxins as well as by traumas, chemical toxins and some other factors. Their progress could be acute and chronicle. In case of Acute periodontists an inflammation liquid is drained /exudate/, but for the chronic periodontists the proliferation of tissue as a protection reaction against the disease causes is typical. Inflammation process is more frequently localized around the edge of the tooth root.
Usually, the periodontitis progresses as a restricted local disease destructing the periodontal coating even sometimes the one of the neighbor alveolar bone, which destroys the tooth function.
Acute apical periodontitis is a complication of the diseases of hard tissues and tooth pulp /caries, pulpits, gangrene/. Microorganisms and their by-products affect the periodontal tissue through the root canals. Type of the inflammatory process depends on the activity of the microorganisms as well as on the resistance of the organism. Acute periodontitis is developed in case of massive infection and reduced resistance of the organism. Frequently, in the practice we could meet patients with gangrenous teeth /pulp of the tooth is necrotic/ without any complaints. In case of acute periodontitis the patient complains of permanent, spontaneous, localize pain increasing during the night. Pain increases when chewing hard food. Typical for the acute suppurative periodontitis are the acute pain, when touching the tooth and pulse pain that disappears only after the draining of suppurative excretion either naturally or after some treatment. Body temperature is sometimes increased up to 380 and the submaxillary lymph nodes are increased and painful when touching them.
Chronic periodontitis progresses as a result of acute periodontitis after draining of the inflammation excrete, reduction of the infection activity, chronic trauma and good resistance of the body. As a result a fibrosis with granulomatous proliferation, localized granuloma, cyst of the root and chronification of the periodontitis could occur. Chronic periodontitis progresses without any special complaints and usually it is discovered after X-ray examination. Granuloma treatment could be either conservative or surgical. Conservative treatment includes mechanical and chemical treatment, disinfection and filling of the root canals. In case of its failure a surgical treatment will be applied, i.e. the granuloma has to be removed and the physiological balance has to be restored.
